Transfer of your OB care to our practice

We are committed to ensuring that you and your baby receive the highest level of care.

Our practice delivers at Chesapeake Regional Medical Center.

The following questions are to make sure we can provide you with the care you need and determine if a higher level of care is needed:
1. Would you refuse a blood transfusion if needed?
2. Is your BMI 50 or greater? https://www.cdc.gov/bmi/adult-...
3. Are you an insulin-dependent diabetic?
4. Do you have Lupus?
5. Have you had a cesarean section in the past and would refuse/decline a repeat cesarean section?

We cannot provide your care if you answered YES to any of the questions above.

Records must be sent before an appointment is scheduled. Records must include:

• Prenatal flowsheet, all lab results (hard copy), and all ultrasound reports (no images needed) for the current pregnancy.

• If any referrals have occurred during the pregnancy (ie, MFM, cardiology, endocrinology, etc), copies of those notes must also be sent. 

Records from out of the country must be translated into English before being sent to our office.

Transfer of care from local providers before 24 weeks will be considered for transfer once the records are reviewed. We do not accept local transfers after 24 weeks.

Out-of-area transfers can be accepted at any time during pregnancy after reviewing records.

Please request that only the current pregnancy records be sent over for IVF pregnancies.
